Why Dentals?

It is that "time of year" when we start talking more and more about dentals and caring for your pet's "oral health" and you may be wondering why?

"Boomer" just went to the vet for his annual check up. He got a pretty good overall review of his health, except Dr. Amber mentioned that his teeth were a grade 2 to 3 and he could use a dental. You are now thinking, what does this mean?

Dogs and cats oral health is graded on a scale from 1 to 4. A grade 1 dental disease is usually very minor tarter with possible areas of some mild gum irritation, but nothing major. Grade 2 to 3 dental disease speaks of areas of inflammation in the gums, moderate tarter, with a possible dead or fractured tooth. Grade 4 dental disease is a mouth that is in severe need of dental care and attention.

Can we make a difference? The answer is YES we can! Dentals at Mountain View are performed completely under general anesthesia with an ultrasonic scalar (just like your human dentist) where we are able to get off all tarter and most staining. We are now able to take digital dental radiographs, so our turn around time on images is within seconds, thus limiting time under anesthesia and waiting. Once the tooth, roots and bone structure can be assessed via radiographs, we are able to determine whether the tooth has to come out or can remain.

Pets do very well after having extractions, in fact, once the extraction sites are healed (2 to 3 weeks post extraction), many pets do even better because they feel better. The thing is, pets can't tell us their mouths hurt or there is pain. Often our only sign is more wear and tear on one side of the mouth verses the other because they are favoring one side over the other.

Dentals can help the pet on so many levels, not only just with the comfort of their mouth. It can prevent heart and liver disease, or further health complications down the road. It is pretty astounding how such an overlooked procedure can actually seriously benefit a pet.

We are big believers of dentistry here at MVVH. We want to educate each and every pet owner to the importance of what a dental can do for their pet and the overall well-being of that pet.

TPLO: Tibial Plateau Leveling Osteotomy

This is "Cheech", a 3 year old neutered Rottweiler that presented to MVVH 9/2015 with a right hind lameness. After a physical exam, sedation and radiographs, it was determined that "Cheech" was in need of a surgical procedure referred to as a TPLO.

To begin, understanding the anatomy is important. A TPLO surgical procedure helps stabilize the "knee" (or stifle) of the dog. In most cases trauma has occurred to the cranial cruciate ligament (CCL), which is the ligament that prevents the femur, or thigh bone, from moving backwards and forwards along the top of the tibia. In other words, it is a very important piece to a normal, pain free function of the knee.

To officially diagnosis the issue, radiographs are taken under sedation to ensure motionless views with correct angles of the knee. A normal range of the knee is compared to the angle at the time of the radiograph to determine what size of implant is needed to reconstruct the angle of the knee to a more ideal and/or correct angle to ensure the future comfort of the pet.

Prior to any orthopedic surgical procedure, blood is collected and ran to confirm the safety of anesthesia with the pet. An intravenous catheter is placed to guarantee security of fluid and drug administration. Once the patient is intubated and the leg is clipped, the patient receives an epidural to deliver pain relief in the hind quarters of the pet.

Once the patient is aligned correctly on the surgical table, draped and all instruments are unwrapped and placed in desired locations, the surgical incision is made along the midline of the knee.

Following the initial incision, the first step is the arthrotomy where the inside of the stifle joint is visually inspected to find and remove traumatized ligament. Additionally, the meniscus will be inspected and if damage is present those pieces will be also removed.

The second step to the surgery is the osteotomy of the tibia, which where the tibia is actually cut with a concentric saw. This enables the rotation of the tibia into a more normal, ideal angle of the knee. The osteotomy is stabilized with a designated plate that has anywhere from six to eight screws to secure the plate in place.

Once the plate is placed and the surgeons feel they have successfully completed the surgical procedure, the patient is taken to radiology while still under anesthesia where a radiograph is taken to verify the placement of the plate and screws to ensure completion.

Aftercare of the surgical procedure can be time consuming with limited walks, mild physical therapy, laser treatment and post surgical radiographs to guarantee continued placement of the plate and screws. The pet should be back to full function of that limb within 10-12 weeks of the surgery.

Why a TPLO? A TPLO is highly regarded as the gold standard of surgical treatment for a torn cruciate ligament. There are other procedures available, but the statistics in the end still favor a TPLO for the best return to normal function for medium and large breed dogs.

A Couple Things...

Hello Friends! We want to introduce you to VINx a program developed for veterinarians by veterinarians. This is an online program that functions for YOU, our clients. We MUST have your email for it to work. You will receive an email and then need to create a password. With that log in you will be able to access your pets' vaccine records, medical procedures that have been performed, request an appointment, boarding dates, not to mention the ability to request prescription or food refills. You will also be able to leave the staff a note regarding any questions or the health status of your pet. The additional neat thing about it is that our veterinarians will be able to access the information from home when on emergency call, allowing them to see what we have done for your pet and a good history of your pet. In the next couple months we will be linking this with our outside laboratories, so the veterinarians can refer to your pets' lab work while out of the office. We will also be able to request a review or survey from our clients allowing us to publish them to our Facebook page or Website. It is obviously a new program to us and to our clients, but we hope with your help we can make it run and work as efficiently as it should!

  The New Building! Progress is going well! We are pretty much on track to be in it in October 2015. We anticipate closer towards the middle to end of October. We have had lots of questions about it and felt we should emphasize a few things! YES....all of our staff is moving with the transition and are eager to share the move with all our clients. ALL client information will be transferred for every individual pet within the family. We worked REALLY hard to make this transition within our current fees, we did not want to have to increase fees because of the choice we made to move. Just the small animals are moving. Rocky Mountain Equine is a separate business than MVVH and that business will remain in our current location. We are looking forward to this transition and even more making it with the clients we have and the ones we welcome. We will have four exam rooms in the new facility, we will have digital radiography, and have the full intent of going paper-light. Dogs and cats will have their own "wards" for surgical recovery or for hospitalization. It was very important to us that your pets could wake up or be treated in a more peaceful environment. Overall, it will be a BIG change, but a change that has been in the works for close to 11 years. We are ready to embrace the changes and hope we can share it with as many people as we can!!

Meet Sherman!

This is Sherman, roughly a 17 month old Boxer mix. He presented to MVVH on 5.18.2015 for a weekend of vomiting and not wanting to eat. Upon a good physical, blood work and an ultrasound it was diagnosed that Sherman had eaten something he probably shouldn't have! His owners gave us the go-ahead to take him to surgery. A foreign body was removed and Sherman returned to recovery. Within two days Sherman still was not feeling up to snuff and vomited again, which we always become concerned about following such a big surgery. Dr. Amber re-ultrasounded him and found that he had fluid in his abdomen and that emergency surgery was going to be required to go back into his abdomen and get rid of the fluid and find out the cause. Sherman had what we call a Sterile Peritonitis (a sterile inflammation within his abdomen), but without fixing the issue and getting rid of the fluid Sherman would not recover well. The owners gave us permission to take him back to surgery. The issue was resolved and Sherman was kept another four days. We had to keep him longer than most foreign body surgeries because we had to make sure he could handle multiple small meals, that he continued to drink and eat without vomiting, that his temperature stay within normal limits and his attitude remained bright and alert. We are happy to report he revisited us on 5.26.2015 and was doing GREAT! Sherman was an intense case that touched all of us, and we are SO happy we got to send him home with a good bill of health!
